2008 Mail-in Summer Camp Application

The mail-in application is available as a PDF .

Please remember that the balance is due 10 days prior to Camp Session.

If your child is attending Junior High 1 or 2, Sophmore, or Senior camp and wants to participate in the Ropes course, you must complete a liability waiver for the ropes course. If so, please complete this form and send it in with your application and deposit.

Applications mailed in less than 10 days till the start of the camp may not be accepted. Contact Michael for availability (michael@campmcdowell.com or 205-387-1806).
